Information for Conducting a Clinic / CWC Policies

Each person receiving an immunization must complete and sign

a consent form. Only those 4 years of age and older may receive

a flu shot at our corporate clinics, ages 6 months and older at our

hospital clinics.

Parents must sign the consent form for those under 18 years of

age. Women who will be pregnant during flu season should

receive a flu shot. We no longer require a physician prescription.

Pneumonia shots are only given to those 18 years and older.

Pregnant women may not receive a pneumonia shot. FluMist will

be available by request and is limited to those ages 5 - 50 and

generally healthy (no underlying asthma, diabetes, no pregnant

women, etc).
